OFFICE HOURS
Please call to schedule your visit in advance. Appointments may be arranged Monday through Friday. 9:00 a.m. to 5:00 p.m. The office is closed on weekends and major holidays.
EMERGENCIES
One of our doctors is always on call (24 hours a day. seven days a week) for emergencies. If you need to see or speak with one of the doctors outside of office hours, please call the office number, 970-243-0900 to be connected with the surgeon on call, or go directly to St. Mary's Hospital Emergency Room.
PRESCRIPTIONS
Approved prescriptions may be filled over the phone at the pharmacy of your choice during regular office hours. For prescription refills, please call your pharmacy first so they may contact us for approval. We do not refill any perscriptions over the phone on nights (after 5pm), weekends or holidays. Please plan to have refill requests made during regular business hours.
FEES
The Surgeon's fees vary according to the procedure performed, and we are happy to answer any questions for you. After major surgery, there is no additional charge for routine follow-up visits. Hospital fees and other services such as lab, radiology or anesthesiology are billed by those providers or physicians. If you need to obtain inforniation on these charges, we can help direct you.
INSURANCE
Please provide us with your insurance information so we may bill your insurance carrier directly. If your policy requires a co-pay, you are responsible for paying this amount at the time of service. Please make sure your primary physician sends any needed referral. If we do not participate with your insurance carrier, you will be responsible for any amount not covered by your insurance. If you are unsure whether your surgeon participates with your insurance plan, please feel free to ask our Billing Department or click here for a list of participating insurance plans.
PAYMENT PLANS
We welcome all patients without insurance, but would like to remind you that you are responsible for payment at the time of service. Cash discounts are available. We also accept Mastercard, Visa and Discover. If you require a payment plan, our Billing Department will gladly customize a plan to fit your situation.
PRIVACY
Traditionally, safeguarding your confidentiality has been one of our highest priorities. In keeping with this belief, we respectfully comply with the privacy and security rules of the Health Insurance Portability and Accountability Act (HIPAA), which were created to provide standards for the disclosure of your health information.
